Chapter 3 - The Doctor's House Call

The rain was falling heavier now, turning the manicured gardens of the Moretti estate into a dark, glistening maze.
In the grand foyer, Dr. Julian Vance hurried inside, shaking the water from his expensive umbrella. He was a distinguished man in his late fifties, wearing silver-rimmed glasses, a pristine trench coat, and carrying a traditional black leather medical bag.
"Bruno!" Dr. Vance called out, his voice laced with orchestrated panic as he wiped his shoes on the entrance rug. "Where is he?! Is Alejandro alright?! Your phone call terrified me!"
From the top of the grand marble staircase, Bruno stepped into view. His hands were clasped behind his back, his face devoid of any emotion.
"He's in the study, Doctor," Bruno said calmly. "He collapsed shortly after drinking his afternoon wine. He's not moving."
Dr. Vance’s eyes flickered with a brief, hidden flash of triumph before he quickly forced a look of solemn grief onto his face. He gripped his medical bag tighter.
"Good God... I warned him about his blood pressure," Vance lied smoothly, hurrying up the stairs. "Let me see him immediately. Do not let any of the house staff enter the room. This must be handled with utmost discretion!"
"Of course, Doctor," Bruno replied, stepping aside to let the physician pass. "After you."
Dr. Vance walked down the golden-lit corridor toward the private study. He took a deep breath, adjusting his glasses, preparing himself to pretend to administer CPR before officially pronouncing the most powerful mob boss in New York dead of 'sudden cardiac arrest.'
He pushed the heavy oak door open.
The room was dim, lit only by the dying embers of the hearth fire. On the floor behind the mahogany desk, a man’s body lay still, facedown on the rug near a pool of dark red wine.
"Alejandro!" Dr. Vance exclaimed, rushing into the room with theatrical urgency. He dropped to his knees beside the body, opening his medical bag. "Can you hear me?! Stay with me!"
He reached out to check the neck pulse.
Before his fingers could touch the skin, the 'body' suddenly spun around.
A large, iron-hard hand reached up, gripped Dr. Vance by his throat, and slammed him backward onto the floor with violent force. The doctor’s glasses flew off, shattering against the marble fireplace.
Dr. Vance choked, gasping for air, staring up into the dark, lethal eyes of Alejandro Moretti.
Alejandro was very much alive. His grip on Vance’s windpipe was suffocating.
"Looking for a pulse, Julian?" Alejandro asked softly, leaning down until his face was inches from the terrified doctor’s. "Or were you checking to see if your three drops of peanut concentrate finished the job?"
"A-Alejandro..." Vance choked out, his eyes bulging with terror, his hands clawing uselessly at Alejandro’s wrist. "I... I don't... know what you're talking about! I came to save you!"
Alejandro pulled the tiny pinhole camera from his pocket and dropped it onto the doctor’s chest.
"Your camera had excellent resolution, Julian," Alejandro murmured. "I particularly enjoyed the frame where you checked your watch after pouring the vial into my Barolo."
Dr. Vance went completely pale. His chest heaved in terror. The refined, respectable doctor vanished, replaced by a desperate, trapped animal.
"Who paid you?" Alejandro demanded, tightening his grip just enough to cut off the oxygen flow. "Was it the Commission? Was it my cousin Marco? Speak, or I will let Bruno test your own medical bag on you."
"M-Marco!" Vance wheezed out, his pride instantly shattering under the threat of death. "It was Marco! He... he has my gambling debts from the underground casinos in Atlantic City! Four million dollars! He promised to clear the debt and give me ten percent of the port profits once he took over the family!"
Alejandro’s eyes flashed with a deep, tragic sadness. Marco was his uncle's son. They had grown up together in Sicily, shared blood, shared bread, and built the empire side by side.
Yet, for money and power, Marco had turned Alejandro's own physician into an assassin.
"Where is Marco right now?" Alejandro asked quietly.
"He's... he's waiting at the marina," Vance stammered, tears of fear leaking from his eyes. "He has thirty men staged on his yacht. Once I called him to confirm your death, he was going to drive here with his captains to 'secure' the estate and execute your loyal loyalists!"
Alejandro stared at the pathetic man beneath him for a long moment. Then, he looked up at Bruno, who was standing in the doorway holding a pair of heavy steel handcuffs.
"Cuff him to the steam radiator in the basement," Alejandro ordered. "And take his phone."
"What about Marco, Boss?" Bruno asked, dragging the weeping doctor up by his collar.

Alejandro walked over to the desk, picked up the empty EpiPen that Emma had left behind, and carefully placed it inside his inner jacket pocket, right over his heart.
"Marco thinks he's coming to an execution," Alejandro said, his voice dropping into absolute ice. "Let us not disappoint him."
