The room fell into complete silence.

No one moved.

No one even breathed.

Evelyn Hart held the small medical cast cutter steady in her hand, her eyes fixed on Noah's swollen arm. She had seen frightened children before. She had comforted burn victims, stitched deep wounds, and stood beside parents on the worst days of their lives.

But this...

This felt different.

Noah wasn't afraid of pain.

He was afraid of what was inside the cast.

"Mr. Vale," Evelyn said quietly, "I'm asking one last time. Do I have your permission?"

Daniel looked from his son to his wife.

Marissa's face had gone pale, though she quickly recovered with a nervous smile.

"This is ridiculous," she said. "He's a child with a vivid imagination."

Noah shook his head violently.

"It scratches at night," he whispered.

His voice cracked.

"It bites me."

The words sent a chill through Evelyn.

Children lied sometimes.

Children exaggerated often.

But trauma taught nurses one important lesson—

Children rarely invented fear like this.

Daniel swallowed hard.

"If opening the cast won't hurt him..."

"It won't," Evelyn answered.

"...then do it."

Marissa stepped forward so quickly that the IV stand rattled.

"Daniel!"

Her voice was sharper than anyone had ever heard.

"You'll expose his fracture!"

Evelyn looked directly at her.

"If the fracture is stable, replacing the cast is routine."

Marissa opened her mouth, then closed it again.

For the first time since Evelyn had met her, she looked genuinely frightened.

That alone was enough to make Evelyn even more suspicious.

She knelt beside Noah.

"I'm going to cut the outside only," she said gently. "You stay very still."

Noah reached for her hand.

His fingers were ice cold.

"Please don't let it touch me again."

"I won't."

The electric cutter buzzed softly.

The blade vibrated instead of spinning, designed specifically to cut plaster without harming skin.

White dust floated into the air.

The room watched.

Daniel stood frozen.

Two nurses leaned closer.

Even Marissa stopped pretending to cry.

Three inches.

Five inches.

Eight.

The cast slowly opened.

A strange smell drifted out.

Not blood.

Not medicine.

Something damp.

Something rotten.

Evelyn frowned.

"That's not normal."

She carefully spread the cast apart.

The cotton padding underneath looked unusually thick.

Almost... intentionally packed.

She slipped on a pair of gloves.

"What are you doing?" Marissa snapped.

"Checking the padding."

"You don't have authority—"

"I do now."

Evelyn peeled back the first layer.

Then the second.

Then a third.

Something moved.

Tiny.

Quick.

She froze.

The movement came again.

Noah screamed.

"It's there!"

Before anyone could react—

A black shape wriggled out of the cotton.

One nurse gasped.

Daniel staggered backward.

Marissa's face lost every trace of color.

The creature hit the hospital sheet and began crawling.

It wasn't large.

Barely two inches long.

Its pale body twisted unnaturally beneath dozens of tiny legs.

A centipede.

Alive.

The nurse crushed it instantly with a stainless-steel tray.

Crunch.

Silence.

Daniel stared in disbelief.

"What..."

Evelyn didn't answer.

She was already pulling away more padding.

Her heartbeat accelerated.

Another movement.

Then another.

"No..."

She ripped open the remaining cotton.

Three more centipedes spilled onto the bed.

One disappeared beneath Noah's blanket.

Another crawled across his hospital gown.

The third emerged directly from a blood-soaked pocket near his wrist.

The room erupted.

Nurses shouted.

Someone pulled Noah away.

Another nurse grabbed disinfectant.

Daniel looked ready to vomit.

"My God..."

Noah buried his face against Evelyn's shoulder, shaking uncontrollably.

"I told you..."

His voice was barely audible.

"I told everybody."

Evelyn wrapped an arm around him.

"I know."

She examined Noah's skin.

Dozens of tiny puncture marks covered his forearm.

Some were infected.

Others looked only hours old.

Whatever had been trapped inside the cast had been feeding on him for days.

Her stomach turned.

This wasn't neglect.

This was deliberate.

She looked up slowly.

Marissa had taken one careful step backward.

Then another.

Almost as though she intended to disappear before anyone noticed.

Evelyn stood.

"Don't move."

Marissa blinked.

"I'm sorry?"

"You heard me."

Daniel turned.

"What is she talking about?"

Evelyn's expression hardened.

"Someone placed live insects inside this cast before it hardened."

"Impossible."

"It is impossible accidentally."

Daniel stared at the exposed arm.

"The hospital..."

"No."

Evelyn interrupted him.

"The cast was professionally applied."

She pointed toward the unusually thick inner padding.

"Someone reopened it afterward."

Every eye shifted toward Marissa.

She laughed.

A little too loudly.

"So now we're accusing family members?"

"No one accused anyone."

"You looked at me."

"Because you're the only person frightened by what we found."

For a split second—

Hatred flashed across Marissa's face.

Pure.

Cold.

Undeniable.

Then it vanished beneath another performance.

"This is insane!"

She burst into tears.

"I've done everything for this family!"

Daniel didn't speak.

He couldn't.

Pieces of memory were falling into place.

Marissa insisting she stay alone with Noah after the accident.

Marissa refusing to let anyone help with bathing him.

Marissa claiming Noah exaggerated pain.

Marissa canceling follow-up appointments twice.

At the time...

None of it had seemed unusual.

Now...

Every memory felt poisonous.

A hospital security officer entered after hearing the commotion.

"What happened?"

Evelyn answered first.

"I believe this child is the victim of intentional abuse."

The words struck like thunder.

Marissa's tears stopped instantly.

"You can't prove that."

"No," Evelyn replied calmly.

"But evidence can."

The dead centipedes were collected.

The cast was sealed inside an evidence bag.

Photographs documented every puncture wound.

One of the doctors arrived moments later, stunned by what he saw.

"I've practiced medicine for twenty-two years," he muttered.

"I've never seen anything like this."

Daniel sat heavily in a chair.

His hands trembled.

He looked at Noah.

Then at Marissa.

Then back again.

Finally, he whispered the question that should have been asked days ago.

"Noah..."

The little boy looked up.

"Did someone do this to you?"

Noah hesitated.

His eyes slowly found Marissa.

She smiled.

It lasted less than a second.

A tiny smile.

Hidden from everyone—

Except Evelyn.

It wasn't comforting.

It wasn't reassuring.

It was a warning.

Noah immediately looked away.

His lips trembled.

"I..."

He couldn't finish.

Daniel reached for his son.

"You can tell me."

Noah began crying again.

"I don't want her to know."

Daniel frowned.

"What do you mean?"

Noah whispered so quietly that only those closest could hear.

"She said if I ever told you..."

He stopped.

Evelyn knelt beside him.

"What did she say?"

The boy's entire body shook.

"She said next time..."

His voice broke into sobs.

"...she'd put something bigger inside."

Every person in the room went silent.

Even the air seemed colder.

Across the room, Marissa slowly tightened her grip around the strap of her designer handbag.

Inside it...

Hidden beneath her lipstick and perfume...

Her fingers wrapped around a small glass container.

Something inside the jar scratched softly against the lid.

She smiled to herself.
