CHAPTER 5: The Masterclass
For the next forty-five minutes, Cath Lab 2 became a cathedral of high-stakes medicine.
Dr. Vance stood by, completely mesmerized, his jaw practically on the floor. The young resident was watching my hands move with a speed and dexterity that seemed almost supernatural for a woman my age.
"Suction," I ordered. "Hand me the prolene sutures. 4-0. Hurry."
The dissection was severe. The tearing of the inner layer of the blood vessel had created a false channel, cutting off the blood supply to Richard's entire left ventricle. His heart was beginning to turn a sickly, ischemic purple.
"He's going into ventricular fibrillation!" the nurse yelled. The monitor went into a chaotic, jagged zigzag.
"Charge the internal paddles to twenty joules," I said, my voice completely devoid of panic. "Vance, hold the suction right here. Do not move. If you nick the atrium, he bleeds out."
Vance swallowed hard, his trembling hands suddenly finding stability under my calm aura. "Yes, Doctor."
"Paddles clear," I said, placing the small metal spoons directly onto Richard's exposed heart. "Shock."
Richard's chest gave a small leap. The monitor continued to scream.
"Charge to thirty. Shock."
Another leap. Still nothing.
"Mom!" Daniel's voice whispered from the viewing gallery window above. I didn't look up, but I felt his presence. I felt twenty-five years of love, of quiet mornings, of Christmas eves, and the crooked angel on the tree. I hadn't given up my career to lose the man who had built that beautiful, quiet life with me.
"Internal massage," I announced.
I slipped my gloved fingers around the back of Richard's heart. I began to squeeze, manually pumping his blood, feeling the rhythm of his life in my bare palm. Come back to me, Richard. Come back.
"Give me 1mg of epinephrine directly into the central line," I ordered. "On three, I'm going to shock him one last time while holding the repair. One... two... three. Shock."
The paddles popped.
For two agonizing seconds, the monitor went completely silent. A flat, terrifying line.
Then...
Beep.
A pause.
Beep.
Beep.
A perfect, beautiful, sinus rhythm bloomed across the screen. The purple hue on the muscle tissue instantly began to flush with a healthy, vibrant pink as the blood flowed freely through my perfectly executed suture line.
"The tear is sealed," I breathed out, stepping back from the table. "The graft is holding. Dr. Brooks, close him up. I need to sit down."
The room erupted into a collective intake of breath. Dr. Vance looked at me, tears of relief in his eyes. "That... that was the most incredible piece of surgery I have ever witnessed in my career. Dr. Monroe, it's an absolute honor."

I pulled off my mask, revealing a tired but triumphant smile. "It’s Dr. Carter, actually. But thank you."
As I walked out of the lab, Daniel was waiting for me in the hallway. He didn't say a word. He just threw his arms around me, weeping into my shoulder. I held him tight, knowing that the family secret hadn't just shattered our past—it had saved our future.
